Is it possible to change destiny?

Is destiny (qadar) already pre-determined? If yes, can’t we change our future mistakes with our acts, cautions or prayers (dua)? Is destiny fixed?

“Allah blots out what He wills and confirms (what He wills). And with Him is the Mother of the Book (The Preserved Tablet of Divine Decrees)” (Surah al Ra’d, 39)

God is free of time and place and He knows everything including our actions and choices with His eternal and ever-lasting knowledge which has no beginning and no end and He has written the destiny on the Preserved Tablet (Lawh al Mahfuz). The Divine Knowledge, which is free from time and surrounds the past, present and future never changes, because it already surrounds anything. However, Allah swt created another kind of tablet which is called “Lawh al Mahw wa Ithbat” (The tablet of erasure and writing) where the sort of destiny which can change under some circumstances is written.  A person can change his destiny written on that tablet with his dua, good deeds and charity (sadaqa).
